 (*Because Dr. Stanton shares the suite with other doctors, be sure you specifically request your appointment with him.)

If this will be your FIRST TIME to SWS and you would like to set up a

new patient appointment, you must read and complete our new patient paperwork. You can print it off directly from this webpage below. To print the paperwork, do the following:

1. Click on the pdf link below that applies to your desired location -  SWS Katy.
2. Select print, print it out, and fill in the required information. (Make sure ALL of the paperwork is filled out) 
3. Fax us your COMPLETED forms to:

You may also send us the paperwork via email (call us for email address), mail, or in person. This will save time and allow Dr. Stanton to review your information before your initial appointment. 

AFTER Dr. Stanton reviews your paperwork, our office manager will call you back to schedule your initial appointment.
